Dementia Cross Sector Pathways

In 2015 a group first came together to consider developing a pathway which is considered to be a ‘blue print’ for dementia care in the North East and North Cumbria. The intent was to do this by building on the NICE dementia pathway and key standards to develop an integrated model to support people’s social, emotional and health needs, putting the emphasis on providing support for all cognitive disorders including people with dementia. The group also wanted to work towards developing a menu of interventions that should be offered at particular points along a person’s journey, understand relevant good practice and consider commissioning and planning implications of the revised pathway and its implementation.

During 2016-17 the project has developed, in partnership with Daniel Carey at Northumbria University. Working with a range of professionals and service users the project evolution has taken in 5 separate workshops and a public exhibition. The final outcome will primarily help patients and carers to navigate available services in their area according to their own priorities. However, it can also be used to help commissioners and providers understand the range of available support and any gaps in provision.
