Diabetic Footcare

Chair: Dr Rahul Nayar, Consultant in Diabetes and Endocrinology and Diabetes Network Clinical Lead

The Diabetes Footcare Network brings together professionals to improve the quality of services and care across the North East and North Cumbria, ultimately reducing major amputation rates. The network members are gathered from the wide spectrum of individual teams that potentially may care for patients with diabetic foot problems. This encompasses both hospital and community based specialists – podiatrists, doctors, surgeons and allied health care professionals – and includes third sector participation from Diabetes UK with active patient representation. The Network is designed to identify and eliminate inequalities by developing pathways of care and sharing good footcare practice and ideas across the North East and North Cumbria.

Key aspects:
a) Encourage early self-referral.
b) Quicker referral to Diabetes Foot MDT leads is associated with less severe ulcers and greater chance of complete healing at 12 weeks.
c) Late referrals are associated with more severe ulcers and a significantly poorer chance of healing at 24 weeks.

Economic Reason for Change

Improving footcare for people with diabetes and saving money: an economic study in England (updated data published January 2017)

Key points:
a) Foot ulcers and amputations now cost NHS £1bn a year.
b) Two thirds of this is spent treating foot ulcers in primary and community services.
c) Savings could be made by reducing prevalence of foot ulcers, and resulting amputations, by ensuring pathways that get people faster to specialist treatment.

Competency in footcare

Key points:
a) All HCP involved with diabetes foot care need to be competent in their practice.
b) If a HCP recognises a problem they should ACT, and if they do not know what to do they should ASK.
